Chronic Gout

Description

Gout is a painful condition which causes sudden and intense pain of the joints along with swelling. The big toe is most commonly affected, but the knee, thumb elbow or fingers may also be affected.

Causes

Gout is caused by an excess of uric acid in the blood. Uric acid is produced by the body and also comes from our diet. Gout is the result of too much uric acid in the blood, either due to increased production or reduced excretion.   The following are common causes of gout:

  • Tendency to develop gout is usually inherited, and may be related to obesity, high blood pressure and diabetes.
  • Hyperuricemia or high concentration of uric acid in the blood.
  • Excess alcohol intake (beer in particular) increases chances of developing gout significantly.
  • Gout can be associated with injuries, certain medicines, high intake of seafood/meat, or any form of stress or injury.
  • Gout may also accompany kidney disorders, enzyme deficiencies, lead poisoning, psoriasis, and some forms of cancer.
Risk factors
  • Gout is more common after the age of 60.
  • Men have a higher risk when they are in between 30 and 50 years of age.
  • Women are at a greater risk after menopause.
  • Overweight and hypertensive men also have increased risk.
  • Taking thiazide diuretics may also increase risk of developing gout.
Symptoms
  • Symptoms include sudden and very severe joint pain. This pain usually occurs at night, or early morning.
  • The affected joint may also become tender and stiff.
  • Swelling and redness of the joint
  • Bone damage may occur if left untreated.
Diagnosis

Diagnosis can be made on physical examination, blood tests and X-Rays.

Management
  • Pain killers are usually prescribed to reduce the pain
  • Non-Steroidal anti-inflammatory drugs such as indomethacin
  • Drugs such as colchicine may be recommended to help reduce the inflammation
  • Allopurinol or febuxostat may be used to reduce uric acid production
  • Lifestyle changes such as the following may help prevent another attack:
    • Drinking a lot of water to flush out the uric acid
    • Exercise and weight control
    • Avoiding alcohol, especially beer.
    • Restricting seafood and meat.
